SUBJECT: Complaint Code/Defect Code
With the introduction of the new Flat Rate Manual (Service Bulletin SW-059-8) it also became a requirement that you must enter the Complaint Code and the Defect Code on the claim (see SW-059-8).
Our initial observations indicate that these required entries are omitted or erroneous in many cases. We also noted that code 99 is used instead of the specific code. The Complaint or Defect Code 99 should only be used in cases where the appropriate code is not listed in the applicable Complaint/Defect Code tables.
Please review your claim completion procedures and take the necessary steps to assure correct completion of these fields.
Please inform all personnel concerned accordingly.
